Core Changing Concepts

Core Changing Concepts (C3) is a human servicing organization developed to meet the unique needs of diverse, multicultural urban communities. C3 is committed to strengthening urban communities by providing effective programs and services, by qualified individuals who live in the communities they serve, focused on the three core areas that affect social determinants of health: the individual core, family core, and community core.

C3 helps to develop healthy, resilient urban communities by addressing adverse community environments that impact social determinants of health such as violence, mental health, lack of education, social and financial capacity, quality housing, and equitable access to resources through education, awareness, advocacy and programs and services. Programs and services offered by qualified professionals who live, work, learn, and play in the same communities they serve is an integral component of developing trusting relationships, which initiates healing and resiliency building processes.
